Outreach and social network service

Service for the hidden elders

With the effort of social workers and volunteers, we look for the elderly who are lack of family support and do not apply for the community care service. We provide them social network and outreach service for them. We also help to distribute social resources to them.

Service object

People who are over 60 years old with the need for the service but do not know how to apply for the community service, have a health problem or face the following situation:

• Lacking the knowledge of the community
• Poor health condition
• Financial problem or poor living environment
• Other special need
• Bad family relationship

Content

• Outreaching Service: reach out to the elderly who have not experienced any social activities of the centre
• Counselling Service: face-to-face counselling and emotional support
• Referral Service: provide a suitable referral service for the elderly
• Connection with local communities: introduce the elderly’s needs and seek for the disadvantaged elderly
